Check out GPS Passion - http://www.gpspassion.com/. It's my favorite source of info on GPS software & hardware for PDAs. They have done an extensive comparison of most bluetooth GPS receivers.
As far as navigation software, I have used Mapopolis & Intellinav (iGuidance, Routis, MyNavigator). My current favorite is Intellinav

Unfortunately, they seem a little out of date. They don't seem to have too many newer bluetooth models.

I'm playing with the Navman BT models right now and so far it works well. These are the current BT GPS packages we've reviewed:

There isn't any device that has these three features built-in. What you probably will need is a smartphone plus a GPS add-on package.

www.anywheremap.com is an aviation GPS software that has a few different options available. They have info on what PDA's they recommend for their software as well as a couple bluetooth GPS units.
